On-site registration fees will be $50 more than the late registration fee above.

Registration Fee Includes Sunday mixer; continental breakfast MTWH; lunch MWH; dinner MW; all refreshment breaks; Tuesday BBQ Dinner; Thursday Closing Banquet and entertainment; a tote bag and final printed program.

Method of Payment Full payment is required with registrations. All registrations must be paid in U.S. Dollars. We cannot accept checks from outside the United States. If you are outside the United States, please pay with a credit card or read the information about wire transfers.

  • Credit Cards: Visa, MasterCard, American Express, Discover, Diners Club
  • Checks payable to Utah State University Conference Services (must be sent by mail with registration form)
  • Purchase Orders (must be sent by mail or fax with registration form)

Confirmation & Receipt

  • If you register on line you will receive an immediate confirmation via email.
  • If you register by fax or phone and provide an email address, you will receive a confirmation via email approximately 3-5 business days after we receive your registration.
  • If you do not provide an email address, you will receive a confirmation via US Postal Service 7-14 days after we receive your registration (allow more time for international mailing).
  • Registrations received within 7 days of the conference without an email address will receive their confirmation at conference check in.

Cancellation Policy Refunds will be made to those registrants who must cancel, less a $75 processing fee. Written cancellation must be received by August 2, 2009. No refunds will be made after that date. Substitutions will be accepted. SIP and Utah State University reserve the right to cancel this program due to insufficient enrollment and limit their liability to registration refunds only.

Disability Accommodations Reasonable accommodations are available for persons with disabilities. Fourteen working days notice is needed to prepare materials and services. When you register, you will be asked to identify your needs and you will be contacted for additional information.

Special Dietary Requirements Special dietary requirements will be provided if requested fourteen working days in advance. When you register, you will be asked to identify your specific dietary requirements.
